Dr Garrido's work, published in Nature Medicine, characterises decidualisation resistance: a failure of the endometrium to prepare properly for implantation and placental development. Using digital tissue analysis, single-cell sequencing and spatial biology, the study found altered glandular morphology in the endometrial tissue of preeclampsia patients, with proliferative-phenotype cells coexisting alongside the expected decidualised phenotype. In other words, the signal that predicts a third-trimester catastrophe is present in the uterine lining before the pregnancy even establishes. MaiRa turns that into a cfRNA signature and a predictive model over a single blood sample. The validation set is the unusual asset here: the PREMOM consortium recruited more than 9,500 pregnant women across 14 Spanish hospitals prospectively, producing 26,000+ samples from 10,000 pregnancies. Prospective multicentre cohorts of that size are years of work and are not something a competitor assembles quickly. CEO Guillermo Ferrando frames the ambition past the single indication: 'Our ambition is not just to build a better preeclampsia test, it is to give every pregnant woman a comprehensive, early molecular picture of her pregnancy.'

Why this matters
